Special Programs

Aetna Better Health has special programs to help members with all their medical, behavioral and social health care needs. Aetna Better Health will work with practitioners and members to help our members get the best care.

Some members have special health care needs and medical conditions. Aetna Better Health Case Management includes nurses who work with many health care practitioners, agencies and organizations to get the services and the care that our members need. Our case management program is designed to help make sure our members understand their condition and treatment plan. Our staff is made up of registered nurses or social workers who have received training in the case management process.

We want to help! Call Member Services and ask to speak to someone on our Case Management team to enroll a patient. Members can choose to join or leave the program at any time.

Aetna Better Health has a Disease Management program for our members with asthma, diabetes, heart failure, coronary artery disease, and COPD (lung disease.) If a member has one of these conditions, they will automatically be put in the Disease Management program. Practitioners can also ask us to enroll members in the program. The Disease Management program can help better manage our members’ health. These programs educate members on disease and give tips on how to stay healthy.
